24-hour Customized Care & Assisted Living
Our 24-hour customized living includes, but are not limited to the following:
• 3 meals and 2 snacks during the day
• Light house cleaning and laundry is done everyday
• Our meal plan is nutritious, healthy and diverse.
• Safety is our priority, change to Safety and elopement risk management.
• Medicine set-up and control is done by Registered Nurse every week
• Daily activity and socialization among client
• Monthly outings for residents’ birthdays change to Individualized cultural, religious and birthday party celebrations.
• We accept Private pay and Medical Assistance please add waiver services CADI, TBI, Elderly Waiver.
• Provide support applying for the MA and GRH
• Resident assessment performed every 90 days to ensure optimum care is provided
• 24 hour care with overnight staff ensure the safety of our resident
• 24 hour on-call Nurse
• Supervisor on site during business hour
• Set-up medical ride and medical appointment and much more

• Arrange Transportation for Medical and Non-medical Appointments
• Scheduling and Coordinating Cares with Physicians and Therapists
• Escort to Medical Appointments
• Mental Health & Behavioral management
• Medication Administration
• Medication Management
• Complex care includes but not limited to:
– Diabetes Mellitus management
– Physical, Medical and mental Health Illnesses
– Intramuscular and Intravenous Medication Therapy on-site
– CPAP and BiPAP Machine Maintenance
– Wound Care
– Indwelling and Condom Catheters Care
